2. Working assessment only. Every patient summary reflects a working clinical assessment based on information available at the time of the consultation only. It does not represent a confirmed, final, or definitive diagnosis or prognosis. Medical conditions can evolve, progress, or present new features at any time. The working assessment described in a summary may change as new clinical information becomes available.
3. Not a medical record. A Ruqelo patient summary is not a formal medical record, clinical note, discharge letter, or legal document. It has no standing as an official record of care or treatment. The authoritative clinical record is held by the treating clinician, institution, or health system.
4. No legal or liability use. Ruqelo patient summaries are expressly not intended for use in any legal, regulatory, disciplinary, insurance, compensation, or liability proceeding of any kind. By accessing a Ruqelo patient summary, you acknowledge and agree that the summary cannot be relied upon as evidence of diagnosis, prognosis, treatment decision, standard of care, or any clinical fact in any legal or formal context whatsoever. This limitation applies to patients, family members, carers, legal representatives, insurers, and any other party who accesses the summary.
5. AI-generated content. Patient summaries are generated by AI and may contain errors, omissions, or information that does not reflect the most current guidelines. They must not be treated as authoritative clinical guidance.
6. Expiry. Patient summaries expire automatically after 7 days from generation. After expiry the content is no longer accessible. This time-limited access reinforces the provisional and point-in-time nature of the information.
